Class evaluation, comment and suggestion form

I would greatly appreciate your feedback on any of my classes.  This form will allow you to voice your comments without identifying yourself so please be candid.  The more I learn from you, the better the instruction I can offer.

 Please use a separate form for each class.  When you are through with your evaluation click the Submit button at the bottom of the form
